Your submitting of a filled Registration Form and the subsequent

The Registration Fee payment is regarded as
“the signing of the contract” between yourself and FIZA CLUB, thereby
giving your permission to FIZA CLUB to hold, process and store your personal information outlined herein.
PLEASE NOTE!
IF at any point your personal information has changed e.g. phone number, address etc. it is the responsibility of the Customer/User/Client to ensure they provide us with the new updated information asap via email. There are no additional charges for updating your outdated personal information.
UCN:
Upon Registration on to our system every client is issued with a Unique Client Number (UCN). The purpose of this UCN is the additional security measure we provide at FIZA CLUB to keep your personal and sensitive information safe, giving you added piece of mind for all your data.
Client personal identification information is stored separately (on our registration system), to your health record created from your appointments; any reports you provide to us to keep on file; and information provided in your Wellness Form. Therefore keeping all your sensitive health information anonymous and only accessible and connectable to you by your health care professional.
All data is stored in a secure and encrypted environment accessible only by your healthcare professional.
WELLNESS FORMS:
Once filled out these forms are downloaded from Google forms and stored in a secure and encrypted environment anonymously thanks to our UCN system. Once the forms are safely downloaded off of Google forms they are deleted from it to ensure your sensitive health information is not on it any longer than absolutely necessary.
PLEASE NOTE, As of 17th MAY 2023, it is no longer necessary for clients to fill in the WELLNESS FORMS prior to the appointments. These forms will now only be needing to be filled out if issued specifically from your health care practitioner for whatever necessary agreed reason.
ZOOM MEETING:
The appointments currently being offered are over Zoom (or other similar platforms) these meetings themselves are never recorded. Any information disclosed in these meetings however, are noted manually for the later studying of the case to allow an appropriate therapeutic treatment plan to be drawn up. These notes normally are typed up during the meeting and stored in a secure encrypted environment or sometimes very rarely if necessary written by hand to be typed up afterwards at the practitioners earliest convenience and the hand written paper immediately shredded once typed up (in the meantime the hand written paper is kept safely and securely and is VOID of any personal identification details).
CASE FILES:
All case files, containing notes of sensitive Client health information noted in the "in-person" appointments and calls, are stored in a secure and encrypted environment accessible by your health care professional only. These case files are anonymous thanks to our UCN system.
DATA SELLING/SHARING:
FIZA CLUB WILL NOT sell or share your data or personal information with any third parties.
The only time FIZA CLUB will share your data is if legally called upon to do so. OR if for your case analysis it is deemed necessary to get a second opinion from a colleague/Supervisor. In said case only the anonymous health information will be discussed in a strictly private and confidential environment and any personal identification information will not be disclosed.
If there comes a need, for whatever reason, where your case files are required to be passed on to another practitioner this will only be done through your direct consent and never will any personal details be passed on without your written consent in an email.
DATA STORAGE TIMELINE:
ACCESSING YOUR DATA:
You, the Customer/User/Client, have a right to requesting a copy of your data that we hold at any point. Please submit your request in writing via email and we will process this request as soon as we can. Please be aware at busy times of the year this may take up to a month. There is no additional charges for requesting your data, if processed by standard service. If you request it URGENTLY (within the week) there will be a fee you will have to incur (£50).
CHANGES TO PRIVACY POLICY:
MEHER ANDLEEB EQBAL, the Owner & Operator of FIZA CLUB and associated social media Platforms Reserves the Right to amend the Privacy Policies Laid out herein at any time, without prior notice and with immediate effect. It is the responsibility of the Customers/Users/Clients to periodically check for any updates to the Privacy Policies and T's & C's to ensure they still agree with them to enable them to still keep using these facilities.
Date of Latest Update is always declared clearly at the top of these pages to ensure ease of spotting latest update date.

- We hold your postal address in the case that we may need to post out remedies to you.

- We hold your personal information and any other client records/data on our systems for the duration that you are registered with us only.
- You are kept on the registration system for up to 10 years from the date of your last appointment/call/case communication with us. Unless there is a legal reason for us to keep you and your data on our system for a different timeframe, in which case you will be notified. Children's records will be kept until they turn 25, or until the 10 year time period has lapsed, where the 10 year time period lapses after their 25th birthday.
- If you wish to have yourself removed prior to this you may do so by requesting it in writing via email. ( There is no charge for keeping yourself on the system or removing yourself from the system.)
- If you wish to keep yourself on the system for longer, this can also be requested by writing in via email prior to the deadline date issued in your de-registration notification email.
- Notification of de-registration gets sent out 2 months prior to your de-registration date.
- The deadline for your email requesting the continuation of your registration with us is 10 days prior to your de-registration date.
- IF you so choose to keep yourself still registered on our system and request so by writing in via email, you and your data will be kept on our system for a further 10 years when the above described system will repeat.
