WHY DO I HAVE TO REGISTER IN ORDER TO USE YOUR HEALTH SERVICES?

In order to provide our health service to you, we need to hold and store your personal information; in-
line with GDPR regulations, we can only obtain and hold this information with your consent and via a secure system. We require your personal information to be able to contact you with our healthcare support, e.g. your recommended nutritional plan.



WHY DO YOU CHARGE £17.50 FOR THE REGISTRATION?

The charge for the Registration fee includes cost for:
  • the process of registering you on to our system.
  • updating your personal details if they were to ever change.
  • de-registering you eventually, as and when that maybe.
  • extracting records and files stored on our system for you, if you ever so choose to have/view it.

WHAT PERSONAL INFORMATION DO WE HOLD OF YOURS AND WHY?

  • We hold your name so that we can connect it to the unique client number on the rest of our records and systems, where you are only identified by this unique number and not your name.
  • We hold your email address as that is our primary source of communication. 
  • We hold your contact number so that we know to take your calls and messages if you need us urgently, (ordinarily we will not be calling or messaging you, unless absolutely necessary).
  • We hold your postal address in case if we need to post out remedies/supplements etc to you.
  • We do not intend to spam your contact number, email or postal address with advertising material, all our advertising of our latest products, services and offers are only ever on our associated social media accounts or on our newsletters - for which you have to apply and opt in separately. We respect your privacy, time and space so we do not believe in spamming our Customers/ Users/Client. IF you wish to be informed about our latest happenings please feel free to follow us on any of your associated social media platforms, sign up for our newsletter or check in on our website periodically.

YES! Please email us to request a pdf version of the  Registration form. However, please note, as this is a pdf form you would have to print it off to fill it in. It can be returned to us via email.





WHAT IS A UCN?

A UCN is the Unique Client Number issued to every Client upon registration into our system. This is part of the added security measures we provide here at FIZA CLUB for your personal information and data. It is emailed directly to each Client.




IF MORE THAN ONE PERSON IN THE FAMILY IS REGISTERED DO WE HAVE THE SAME UCN?

No each registered Client has there own UCN and needs to use only there own issued UCN where necessary.




WHY DO I NEED TO FILL IN A WELLNESS FORM?

The Wellness Forms have been designed to help collect some health information prior to the appointment to allow a rough direction of the case to be mapped to enable more targeted questions and issues to be tackled in the actual appointment time, streamlining the process to make efficient use of our time together. 




IF I DO NOT WANT TO FILL IN THE WELLNESS FORM ONLINE IS THERE ANY OTHER WAY TO FILL THIS IN?

YES! Please email us to request a pdf version of the Wellness Profile form. However, as this is a pdf form you would have to print it off and fill it in. It can be returned to us via email. (PLEASE NOTE: these forms are between 42-53 pages!)

DO I HAVE TO ANSWER ALL THE QUESTIONS IN THE WELLNESS FORM?

Although it would be very helpful if the forms could be filled out fully with sufficient details, however, we do understand that this sometimes can be difficult with disclosing some personal sensitive health information on a form that is out sourced (FIZA CLUB uses Google Forms platform for this purpose). Therefore we do accept that sometimes some sections although maybe needing to be filled out will not be. In this instance we suggest instead of just simply leaving blank or saying N/A, it may be more helpful to say "to be discussed", (or the likes) so that it alerts the section/question for the health professional to enquire about in the appointment.  




WHY AM I BEING ASKED ABOUT MY FATHER'S HEALTH OR MY HOBBIES ON THE WELLNESS FORM WHEN ALL I WANT IS MY ACNE CLEARED? IS IT REALLY NECESSARY FOR ME TO ANSWER THIS FOR MY ACNE?

Sometimes some question may appear irrelevant to the client based on what they think is their health issue. However, our body is all inter-connected and things which one may think is "irrelevant" may be relevant in a holistic way. So please bare with us and trust the process!




CAN I PAY BY BANK TRANSFER FOR THE PAYMENTS INSTEAD OF A CREDIT CARD?

Yes! Get in touch with us via the contact form on our website or email. We can provide bank transfer info and book in your appointment.






ARE THERE ANY HIDDEN CHARGES?

There are no hidden charges from my side. The only "extras" that you may have to pay for are lab costs for tests that you may want, ( e.g. Hair tissue mineral analysis, HTMA, test, GI-MAP test etc.) and any remedies/vitamins that you may require.
- Just like allopathic practices, you get the recommendation from your care provider and have to acquire the recommended "aid" for healing through the pharmacies/health food stores yourself.




DO YOU HAVE ANY CANCELLATION CHARGES?


IF you miss your appointment slot or do not cancel/reschedule at least 48 hours prior to you appointment, you will loose your appointment fees.




WHY DO YOU NOT PROVIDE THE REMEDIES/VITAMINS RECOMMENDED?

Three reasons:
  • This is more supportive to homeopathic pharmacies;
  • One may prefer a different brand of vitamins;
  • For overseas clients, to ensure cost effective, convenient and swift access to remedies/multivitamins with minimal air miles, it is best to order oneself from pharmacies/health food stores that are as local as possible to oneself.

If one does not have any local pharmacies/stockists we can point one in the direction of some reputable pharmacies/stockiest that ship out globally.
 
The only "healing aids" we post out if needed are, tissue/cell salts, as they are uniquely tailored to each individual and not readily available in these exact mixes conveniently from pharmacies, these cost £17 per (30ml) bottle to dispense from our Tissue salt pharmacy (NOT including postage & packaging).

However, occasionally for various reasons we do sometimes post out homeopathic remedies, the cost of these will vary as to the nature of the remedy and the recipients of which will be invoiced for them accordingly.





HOW LONG AFTER MY APPOINTMENT WILL MY THERAPEUTIC PLAN BE EMAILED?

Your therapeutic plan should be emailed out to you ordinarily in 10 working days, but please allow 20 working days for busy times. You will be emailed to inform if due to busy times the plan will take longer than 10 working days. If you have been advised to take a test, this time frame will come into play after the results have been received. IF for any reason there has been a delay you will be emailed to notify in regards of the delay and the time frame to expect your plan in.






HOW LONG WILL I REMAIN REGISTERED WITH YOU FOR?

I will keep your records and keep you registered on my system for up to ten years since your last appointment. After this point all your records will be removed/deleted and destroyed. In the case of Children, their records will be kept until they turn 25, or till ten years since their last appointment if that has not yet lapsed. 

You will receive an email alerting you 60 days prior to the removal and a second email informing you of the completion of the removal.

If you wish at any point prior to this to have your records removed and destroyed you may chose to do so by requesting it in writing via an email to info@fiza.club and it will be arranged free of charge at the earliest convenience.

If you wish to be kept on the system for longer you may also choose to do so, again by writing in via email to info@fiza.club . This email should reach us by the stated deadline in the "removal alert/ de-registration email" (deadlines are 10 days prior to records removal date). If you choose to do so it will be kept for further 10 years, before this process repeats. 




IF I BECOME UNREGISTERED WITH YOU CAN I COME BACK AND STILL RE-REGISTER?

YES! If you have not needed to use my services and you become unregistered after the 10 year period; or you choose to have yourself unregistered at any point, but later wish to return you are always welcome. Please however, bare in mind as you would have been removed from my system, you will need to be regarded as a new client.

WHY DO YOU NOT OFFER ACUTE SUPPORT?


FIZA CLUB no longer supports the provision of Acute appointments. Please contact the Homeopathic Helplines for assistances in Acute situations. This change has been made to support more sustainability to these dedicated acute care services, as well as give space for Meher Andleeb Eqbal to have more dedicated time to focus on her niche - mental/emotional wellbeing, atmospheric influence reflection/assessment and recalibration.




CAN I REQUEST A SPECIFIC THERAPEUTIC "TOOL" THAT I WISH TO AVAIL IN MY APPOINTMENT, LIKE HTMA TEST FOR EXAMPLE?
 
YES, you may request for a specific approach or tool or test for your appointments at any time, this will always be taken into consideration! However, if the practitioner feels that you may require a slightly different line of action, the pros and cons of both will then be discussed and the choice then left to you, the client, as to which way you wish to go after the discussion. THIS IS YOUR HEALING JOURNEY, you are always in full control of it, in the appointments and outside of them.
